  • Beachwood Membership Redemption I/we hereby cancel our membership effective for the upcoming season. Please issue a refund of our member equity ($430). Refunds of member equity will be issued via check in the name of the adult member(s) on the account and mailed to the address listed in the member portal. Please make sure your address is correct. Refunds will be issued when the annual membership process is complete which is typically after April 1st. By completing the form below, you are effectively providing your signature, indicating that all the information on this form is true and accurate, to the best of your knowledge.
